Once a Short Breaks assessment has been completed, it could be decided that agency support is needed. An agency from the following list will be contacted to support a CYP and their family.

Agency support will be in one of the following ways:

-        Support in the mornings and evenings with dressing/undressing, feeding, washing

-        Support at weekends taking the CYP out in the community

-        Community based support – providing a travel companion to and from activities

-        Travel training support

-        Support in the community to build independence
