Kaleidoscope Palliative Care Service
The Kaleidoscope team is delivered by the Specialist Nurse for Children with life limiting or life threatening conditions and the Community Play Specialist, in partnership with the Children's Community Nursing Team , tertiary hospitals, Symptom Care Team at Great Ormond Street Hospital and KaleidoscopeTeam .
This specialist team along with the Multi-Disciplinary Team (MDT) including the paediatric dietician, physiotherapist, Social Services, education, and Children's and Young Persons occupational therapist ensure that the child and family's needs are managed within the community setting.
Kaleidoscope aims to provide a unique and dynamic service for children with life limiting or life threatening conditions. This is achieved by working in in conjunction with other services to ensure that the holistic needs of the children and their families are met.
End of Life care for children in CLCH is delivered against the Core Care Pathway for Children with life limiting and life-threatening conditions , which is divided into three stages, comprising of six standards which specify the level and quality of care that every family should expect.

Who is this service available to?
Kaleidoscope is designed for children from 0 - 18 years who have a life limiting or life threatening condition.
The areas that Kaleidoscope covers are Kensington and Chelsea, Westminster and Hammersmith & Fulham.
When is this service available?
Children's Community Palliative Care Nurse available 08:30 to 17:00, Monday to Friday.
Our palliative children’s service is one of the very few Children Community Nursing services who provide 24 hour service towards the end of life.
How do you access this service?
Children can be referred by their GP, tertiary hospitals, health visitor, school and other healthcare professional.
