Our district nurses work in people’s homes and the local community to help patients improve, maintain, and recover their health, cope with health problems, and achieve the best possible quality of life, whatever their disease or disability. Every year our teams help thousands of people maintain their independence and support people return to their own homes sooner after a hospital visit.

Our district nursing service is for housebound people aged over 16 who require nursing care in their home and local community.
How do you access this service?
People can be referred to our district nursing service by their GP, hospital consultant and other healthcare professional. Patients and their carers can also self-refer to our district nursing service.
