A nurse led team of care workers who provide respite care and support to children and young people at home, following detailed assessment of the child’s needs and providing they meet recognised criteria. The care workers are all trained to provide specialist care and play tailored to individual children, young people and their families.

Children and young people who live in Kensington & Chelsea, LBHF, or Westminster, who meet the criteria for the Kaleidoscope Palliative Care Service.
- When is this service available?
Care Worker hours are arranged following assessment, and in agreement with families. Hours may be provided during the day or evening, and at weekends.

Via the Kaleidoscope Palliative Care Team
- How do we ensure the quality of this service?
We are constantly gathering feedback from families on the care they are receiving. This will be done either through a paper form, or on an electronic tablet. Families, children and young people are offered the opportunity to complete feedback on a regular basis.
Patient Story interviews are done with a number of families each year. This is an opportunity to explain how the CCN Service has supported the child’s care, and for parents and children to make suggestions as to how this could be improved.
