Our community nursing teams care for people in their homes, and healthcare and community centers across the wider community. Our community nursing service aims to:

  • Promote health and wellbeing by supporting people to make healthier choices.
  • Manage long term conditions by improving quality of life and independence.
  • Avoid unnecessary admission to hospital and accident and emergency attendances.
  • Provide end of life care by identifying patients who are approaching the last twelve to six months of their life, discussing with them their

Who is this service available to?

Our district nursing teams provide care for patients that fall under three categories of needs.



  1. Regular care interventions, such as daily insulin administration, PEG feeds and administration of daily medication.

  2. Short term care interventions, such as phlebotomy.

  3. Intensive support care interventions, such as end of life care, IV antibiotic therapy and managing pain control. Patients who are identified as end of life may require care interventions at all three care categories. advanced care plans with them to achieve their preferred place of care.

Referrers can refer to our district nursing service through our borough based community nursing referral hubs.
